Welcome to the instruction page for the online forms required by UIL and Waller ISD.

These forms are required each year your son/daughter wishes to participate in athletics for Waller ISD. The athletic department would like to thank you and your son/daughter for filling out the forms and submitting them to us in a timely manner.

To get started, you will need the student’s name and student ID number.

Directions:

1. The required annual Physical and Medical History cannot be filled out online. It must be completed by the parent/guardian and physician. You may print a physical and medical history by clicking Download and Print and selecting the Pre-Participation and Medical History Form.

2. All other required paperwork will be done electronically. Parent/Guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically.

*Band students must complete all of the forms, everyone else should complete all forms except 'UIL Acknowledgement (Band Only)'.

3. Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not be accepted electronically without all required information being completed.

4. Click HERE for instructions on how to create a Parent Account.

5. To access the online forms, select “Proceed to Online Forms” below. To begin, login using the account you created in step 4 (you may also choose to continue as a guest if you do not wish to create an account).

6. Click on the form name and fill out the information requested. Parent/Guardian must have their son/daughters present along with the student’s ID number available to complete the forms. If you do not know the answer to a question, please answer the question with N/A, or none.

7. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD/Tablet/Cellular device, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.

8. Once you have completed the form, you will have the opportunity to print the document for your records. You are not required to print and return forms to your child’s campus.

9. Complete steps 6-8 for each additional form (Again, Band students must complete all the forms, everyone else should complete all forms except 'UIL Acknowledgement (Band Only)').
